This record should be on each report for every Plan 3 member's retirement contribution or DCP participants' deferral information the first time you report the member or participant. The Defined Contribution Record, the Member Profile Record and the Plan 3 Choice Record must be reported the first time you report a member transferring to Plan 3. See layout details below.
